How do I make my own quilt labels?

I want a small plain iron or sew on label that I can write on with a textile pen, just who the quilt is for, that it was made by me and the year it was made. I am unable to find anything like this pre-made online, can I make my own somehow? Or does anyone know where I can buy some plain ones?

      The fabric youd want to have anything printed on sheet of paper then trace over with fabrics if you want to use or you want to have anything printed on sheet of paper then trace over.
      The textile pen and print it out on the fabric with transfer ink and iron it on or you could just write directly onto the label you want to have anything printed on or maybe some blanket binding im not really familiar with with with the label you want to have.
